Ancient Crisis Fragment only drops from Count Geonor in the final room of the Iron Citadel. It does not appear in campaign zones or regular maps. Focus on unlocking tower-shaped structures on the Atlas to access Citadel encounters.

Do I need to defeat both lieutenants before Count Geonor spawns?

Yes. Each Iron Citadel contains two lieutenants that must be defeated before the final room unlocks. Skipping them will prevent the boss from spawning and block Crisis Fragment acquisition.
