Can Herald of Plague spread multiple Poisons at once?

No. Only the most damaging Poison instance is spread per kill, not the full stack or multiple sources.

Does the spread Poison retain its original duration and scaling?

Yes. The spread Poison keeps the same duration and damage scaling as the original instance, including any modifiers from gear or passives.

Does the Hinder effect stack or refresh?

Hinder does not stack but can be refreshed. Each successful spread has a chance to reapply or refresh the Hinder duration.

Can this skill trigger on Poison kills from traps, totems, or minions?

No. Only Poisons applied by your own non-Herald attacks will trigger the spread effect. Minion or trap kills do not count.
