Can Fox Idol be removed after socketing?

No. Once socketed into a weapon or armour, Fox Idol become permanently bound. You can overwrite them with another socketable item, but the original Idol will be destroyed.

Does Fox Idol affect all skills or only socketed gems?

The +2% to Quality of all Skills applies globally to all active skills while equipped in body armour—even those already at 20% quality. This bonus stacks additively and benefits any skill affected by quality modifiers.
